Dear colleagues, welcome to my channel again, it's been a while since I uploaded a video. This time it is about glaucoma.
A 35 year old lady came with a poor IOP Situation and a complicated Cataract with synechiae after having multiple glaucoma surgeries in Russia since birth. Those surgeries included 2 not working Trabeculectomies, so the decision was not easy. Because of the fact that the patient was young, we decided not to do a IOP-lowering procedure which will destroy the ciliary body and threat the vision by causing a hypotony. By wanting to treat the cataract and the glaucoma at the same time, we went for a phacoemulsification with an in the bag IOL-Implantation combined with a XEN-Stent-Implantation (MIGS plus). Some pearls for the surgeons in this surgical case are:
1)Megalocornea of 15mm
2)Zonular insufficiency at 11 o clock with vitreous prolaps
3)Rigid small pupil treated using the maluygin-ring
